Two islands, warm reefs, and easy boat diving in Malaysia's Marine Park waters

Perhentian is generally a low-stress destination, but the two real risks are the sea (monsoon and boat traffic) and limited on-island medical capacity. Plan conservatively, follow your operator's call on conditions, and treat Marine Park rules as non-negotiable.
Medical resources on the islands are limited. For serious injury or illness, evacuation is typically by boat to Kuala Besut, then onward by road to mainland facilities. For diving-related emergencies, your dive operator will coordinate first aid and evacuation logistics. If you dive, carry dive accident insurance (for example DAN or equivalent) and keep your insurer's emergency number offline on your phone.
Northeast Monsoon sea crossings
From roughly November to February, rougher seas and higher winds can make boat travel hazardous and lead to canceled transfers. Do not force a crossing if conditions are unsafe.
Boat traffic near beaches and jetties
Water taxis and tour boats are constant in peak season. Avoid snorkeling across boat lanes, use a float for longer swims, and surface cautiously on dives.
Fire coral, urchins, and cuts
Shallow reefs mean you are close to stinging and sharp life. Good buoyancy, a rashguard, and careful finning prevent most incidents.
Dehydration and heat stress
Boat days plus tropical humidity add up. Drink water between sessions and add electrolytes, especially if you freedive or do multiple dives per day.
